Oversee, develop and facilitate transition program for students with disabilities.

DUTIES AND RESPONSIBILITIES:  

Orchestrate transition planning with students, YTP staff, parents, school district personnel, related support services and appropriate adult services, providing peer mentoring and advocacy to students with disabilities.
